Seeing Patients Up to Age 18

Revised Age Limit Policy

We are reaching out to the families of our older patients to inform them of a change in our policy regarding the patient age limit at our practice. We will begin the process of transitioning our patients to an adult provider at their 18 year well visit. Their 19 year well visit will need to be with their new adult practice. Our office may make exceptions to this age limit policy for our very complex patients.

As our 18 year old patients transition into adulthood, we feel that they should establish care with a provider that will be able to see them continuously as they mature. We understand that the process of choosing a new provider can be overwhelming and that many offices are closed to new patients. Our office will provide our older patients with the resources and support they need to transition to an adult PCP. This includes a continuously updated list of adult providers in our area with open practices, the necessary release form to transfer their records to the new practice and support staff that can help our older patients connect with a new office.

Our office will be mailing out the transition paperwork in advance of the patient’s 18 year well visit with us. We are asking our families to review this paperwork and select a new provider before that well visit. Many adult practices are booking well in advance, and we want to ensure that our patients will have the time needed to book their 19 year well visit with their new adult provider. Pediatric & Adolescent Medicine will provide continuity of care until that first new visit, including sick visits, follow up/recheck visits, and medication refills.

At this time, our office will honor all previously scheduled well visits beyond 18 years. However, we encourage our older patients to start this transition process now to make sure that their next well visit can be booked with an adult provider. In the event that you are able to establish with an adult provider and do not need your next visit to be at our office, please call to let us know.
